I’ve been hearing about inflammation non-stop for the last two years, and so many cardiologists I’ve talked to were eager to see it become a therapeutic target. Those cardiologists will be disappointed with ZEUS. The lipid-lowerers can now say “I told you so.”
A big thanks to Novo for disproving this hypothesis. Now we’ve all got a few new wrinkles on our brains.

Cardiovascular inflammation has been the talk of the town in cardiology, and while Novo Nordisk was wise to pursue treating it, results from the ZEUS trial have led to a thunderous debate on whether or not it ever played a role in ASCVD.
- The ZEUS trial tested ziltivekimab, a monoclonal antibody that targets the IL-6 pathway to reduce inflammation.
- This trial design was built on the theory that lowering cardiovascular inflammation would translate into fewer CV events.
To test the inflammatory hypothesis directly, ZEUS enrolled more than 6,300 patients with ASCVD, CKD, and elevated inflammation (hs-CRP ≥2 mg/L), randomizing them to once-monthly ziltivekimab or placebo.
- The drug reduced both IL-6 and hs-CRP levels, which confirmed its ability to engage with those molecules.
- But this didn’t reduce the composite of CV death, nonfatal MI, or nonfatal stroke (HR: 0.99), which is a result about as flat as a trial can produce.
- Also, unsurprisingly, inhibiting IL-6 led to severe infections in some patients.
The result immediately reignited a running debate over whether inflammation is a treatable cause of cardiovascular events or merely a risk marker.
- Some cardiologists question whether ZEUS marks the end of the inflammatory hypothesis, while others argue lipids should have always been the MACE focus.
- Furthermore, the ACC’s October 2025 statement urging docs to factor inflammation into risk assessment looks less applicable than before.
It’s important to remember though, the broader anti-inflammatory story is mixed rather than uniformly negative.
- Colchicine, backed by the LoDoCo2 trial, became the first anti-inflammatory drug FDA-approved for CV event prevention in 2023 and is widely endorsed.
- On the other hand, the 2024 CLEAR SYNERGY trial found that colchicine failed to reduce MACE, meaning even it has an inconsistent track record.
However, Novo Nordisk isn’t abandoning the molecule. Two other ziltivekimab trials, HERMES in heart failure and ARTEMIS in post-MI patients, will continue as planned.
- Those trials test different populations, leaving open the possibility that IL-6 inhibition helps in settings other than the ASCVD/CKD group.
The Takeaway
While this trial isn’t a win for Novo, it’s a win for science. When a well-designed trial’s results are this flat, they help truly answer the hypothesis of whether or not reducing inflammation decreases MACE risk. The answer? A resounding “no.”

- Attruby Defends Its Turf in ATTR-CM: A new sub-analysis of the ATTRibute-CM trial bolsters the quality-of-life case for BridgeBio’s Attruby (acoramidis). Digging into KCCQ-OS scores from the drug’s Phase 3 trial, researchers found that after 30 months, Attruby patients were more likely to be “alive and well” than those on placebo (46% vs. 31%), with 2.1 times higher odds of being “alive and better.” The data arrives as the ATTR-CM market crowds and as recent results favor stabilizers over silencers, following the failure of AstraZeneca/Ionis’s eplontersen (Wainua) trial.
- Mitral Repair Outcomes by Sex: Oftentimes in cardiology, outcomes aren’t level across men and women, but it looks like that isn’t the case in M-TEER. Across a study of 65k patients admitted for M-TEER (45.8% women), overall in-hospital mortality was 1.86% with no imbalance between sexes, and rates of stroke, MI, and vascular complications were similar. However, older women fared worse on cardiac tamponade (aOR: 2.05), major bleeding (aOR:1.23), and transfusion. Those results likely stem from women’s smaller vascular and annuli.
- Endotyping the ANOCA Patient: A new registry study suggests that invasive coronary functional testing can “endotype” angina with non-obstructive coronary arteries into distinct categories. Among 525 patients who were endotyped, medication prescription patterns differed significantly (i.e. epicardial, microvascular spasm, or normal physiology). For example, more beta blockers, ranolazine, and RAAS blockers were prescribed for microvascular dysfunction endotypes and more calcium channel blockers in vasospastic ones. This kind of research is a good reminder as to just how intricate treating ANOCA can be.
- Dapagliflozin Works Across the HF Continuum: Helping settle lingering uncertainty about its effect in the inpatient setting, a new meta-analysis found that dapagliflozin consistently improves HF outcomes whether started in hospitalized or ambulatory patients. Pooled data from three trials (n=13.4k) showed that dapagliflozin cut the risk of cardiovascular death or worsening HF by 22% (HR: 0.78). That effect held consistently across all three trials and every subgroup examined, including hospitalized versus ambulatory status (23% vs. 19% relative risk reduction).
- P&F Buys Into Mitral Repair: Austria’s P&F Products & Features acquired AVVie, adding the company’s AngelValve Augmentation System for mitral regurgitation to its structural heart portfolio. P&F says pairing AVVie’s R&D infrastructure and intellectual property with its own development and commercial capabilities should shorten regulatory timelines. The deal targets more than 13M people across Europe and the U.S. who have mitral regurgitation, with no more than 5% of those with moderate to severe disease currently receiving treatment.
- A Calculator for the 2027 Fee Schedule: The ACC created a downloadable 2027 Medicare Physician Fee Schedule calculator to help cardiologists estimate how proposed payment changes could affect their practices. The ACC notes that overall reimbursement for cardiovascular services is projected to rise by about 1% versus 2026, though the PFS changes’ impacts will vary depending on a clinic’s patient populations and service mix. The spreadsheet-based tool spans five tabs, including CPT code descriptions, a 2026-versus-2027 fee comparison, geographic cost indices, and the RVUs used in both years.
- Build-a-Blood Vessel: What if it was possible to craft a custom blood vessel graft for a patient who’s already on the operating table? Well, it may soon be, per an Advanced Materials paper. Harvard researchers have figured out how to spin liquid polymers fast enough to turn them into grafts in just minutes. Next up, a disappearing act: the patient’s tissue regrows around the implant, and the graft degrades. Now, researchers are looking to scale tests in large animal models.
- Another Fall for Ascend’s Blood Thinner: Some batches of dabigatran etexilate, Ascend Laboratories’ Pradaxa generic, have been recalled by the FDA due to impurities. More than 5k 75- and 150-mg capsules of the blood thinner used to prevent stroke and blood clots are included in the recall, all with an April 2027 expiration date. Ascend had another run-in with recalls for the same drug in 2023 when high levels of N-nitrosodimethylamine, a potent carcinogen, were detected in packs of the anticoagulant.
- Hard-to-Beat Hospitals for Heart Care: NYU Langone came first in U.S. News & World Report’s 2026 rankings of the best hospitals for cardiac care. In fact, New York is home to three of the top five hospitals (NewYork-Presbyterian Hospital #2, Mount Sinai #5) for cardiology. Based on data from patient outcomes and expert opinions, Mayo Clinic (#3) third and Cedars-Sinai (#4) round out the top ranks. A 2023 survey showed 94% of respondents take this data into account when choosing care.
- Imricor’s Making Radiation-Free Cath Feasible: Following Imricor’s FDA clearances for its Vision-MR diagnostic catheter and NorthStar mapping system, the company is going all in on heart care with the launch of Imricor Cardiology. The division is focused on helping hospitals replace separate X-ray and MRI sessions with a single interventional magnetic resonance lab, decreasing procedural time by 35%. This builds upon Imricor’s goal of expanding access to radiation-free catheterization, especially in areas like pediatric care which often involve repeated procedures (and increasing exposure to radiation).
- Lp(a) Could be Linked to VTE: But that link depends on sex hormones, according to an EHJ study of 373k men and women. Lp(a) levels ≥125 nmol/L were associated with a 32% increased risk of venous thromboembolism — but only for pre-menopausal women. Researchers traced this back to circulating estradiol. When estradiol levels doubled, the Lp(a)-VTE link increased by 46%. The authors note that factors like hormone replacement therapy or birth control could further amplify VTE risk.
